Dear parents,
 
The NYC DOE and the Office of School Health work together each year to provide students with the services they need to fully participate in school.  Every school year new forms are needed to be completed by your physician.  If your child needs health services, medication, or accommodations please click on the link below to print the forms needed for your child.  Please have your MD complete the front of the form; parent signature required on the back of all forms; and send the forms to one of the appropriate addresses below.    No forms should be returned to school, all forms must be sent to one of the addresses below  (most forms will be sent to the Director of Nursing address; only Diabetes forms should be sent to the DMAF Coordinator).  Please remember to sign the back of the forms and keep a copy for yourself.
