  • Welcome to The Office of School Health! 

    My name is Nurse Cindy & I would like to extend a warm welcome to all students & their parents.  I look forward to working with you & your children at PS/IS 49 during the 2016-2017 school year.  To ensure your child's health & safety, please use this website as a source of information to obtain various forms & updates needed for this school year.


    No student is to take or carry prescription or over the counter medications to school.  If a child by order of an MD needs to receive medication in school a medication administration form must be completed & signed by a physician.  A parent's signature is required on the back of the form.  This form along with the prescribed medication in it's original container must be brought to school by the parent/guardian.  All medication will be administered in medical room only unless the MD ordered for the student to self-carry/self-administer their medication.

    All medications must be picked up by a parent/guardian on the last day of school on Wednesday, June 28, 2017. 

    Medication forms can be found under the Related Links by clicking OFFICE OF SCHOOL HEALTH FORMS.  Please print the appropriate forms, have an MD complete, & return to school.
